Creating Flexible Exposures in Treating OCD and Related Anxiety Disorders using Acceptance and Commitment Therapy

Content: Exposure Response Prevention (ERP) is the frontline treatment for OCD and all anxiety disorders. Current data supports that both ERP and an Acceptance and Commitment Therapy ACT/ERP approach are effective treatments for OCD sufferers. In this presentation, the interaction between the inhibitory learning model (ILM) for ERP and ACT-based exposure are briefly discussed as an introduction to the nitty-gritty of how to facilitate an ACT-consistent exposure session in the treatment of Obsessive Compulsive Disorder.

This workshop will focus on teaching participants how to conduct a contextual-functional assessment for OCD and related conditions, how to facilitate an ACT-consistent exposure session, and what to do when getting stuck in an exposure session (e.g. clients have a high degree of believability on thought-action-fusion, exposures turn into rituals, relatives inadvertently reinforce OCD, the costs of avoidance outweigh the gains of living a meaningful life, etc).

In addition, the Choice Point, an ACT-derived behavioral intervention, will be presented as an additional case formulation and treatment intervention to augment the impact of ERP sessions for pediatric OCD. This includes an experiential exercise to see the choice point in action when working with OCD to help participants to (1) identify client’s hooks and helpers, (2) clarify client’s values, and (3) discriminate values-based behaviors from OCD-driven behaviors.

Objectives:

-Describe how ACT capitalizes on the inhibitory learning model to shape exposure-based interventions for OCD
-Harness valuing and committed action as processes to shape willingness to engage in exposure-based work
-Apply ACT components of the hexaflex to augment ERP sessions
-Use the “choice point model” as a clinical tool to discriminate values-based behaviors from OCD-driven behaviors when working with clients struggling with OCD
